Source Note
A photocopy was found in the papers my Auny Mary Bouschard sent to my father after my grandmother died in the early 1990s. The page appears to have been torn from the bible of William M. Forsyth and appears to have been started by him when his first son was born. The entry for the first three are in his handwriting. Further evidence is that he wrote his wife's birthdate as January 22 instead of January 21. The rest of the entries are written by another hand, presumably Maggie Scully's.
